    <p>Мигрень — второй по значимости источник инвалидизации в мире и первый среди молодых женщин. Несмотря на впечатляющий прогресс фармакотерапии последних десятилетий, у части пациентов приступы продолжаются вне зависимости от лечения. Поиски альтернативных путей привели к развитию хирургического лечения этого сложного заболевания. В нашей статье прослежена эволюция хирургии мигрени — от неолитических трепанаций до современных имплантируемых нейростимуляторов. Показано, как менялось понимание природы болезни и как каждая новая теория патогенеза порождала новое поколение операций: большинство из них потом были оставлены, некоторые — пережили проверку временем. Отдельно разобраны современные данные о декомпрессии периферических нервов, стимуляции затылочного нерва, крылонёбного ганглия и блуждающего нерва, а также неинвазивных методах нейромодуляции.</p>

   <trans-abstract xml:lang="en">
    <p>Migraine is the second leading cause of disability worldwide and the leading cause among young women. Despite the remarkable advances in pharmacotherapy achieved over recent decades, some patients continue to experience migraine attacks regardless of treatment. The search for alternative therapeutic approaches has led to the development of surgical treatment for this complex disorder.&#13;
&#13;
This article traces the evolution of migraine surgery, from Neolithic trepanation to modern implantable neurostimulation systems. It demonstrates how the understanding of the disease has changed over time and how each new theory of pathogenesis gave rise to a new generation of surgical procedures. Most of these procedures were subsequently abandoned, whereas some have withstood the test of time.&#13;
&#13;
Particular attention is given to current evidence on peripheral nerve decompression, occipital nerve stimulation, sphenopalatine ganglion stimulation, vagus nerve stimulation, and non-invasive neuromodulation techniques.</p>
